What types of cardiovascular services do you provide?

What are your clinic/office locations?

We have four offices for your convenience. Find out more about our locations, hours, and parking information.


How do I make an appointment?

Call the specific Northwest Cardiovascular Institute clinic that you prefer. The front desk person there can schedule your appointment for that location.


Do I need a referral from my primary care doctor?

No.


What types of insurance do you accept?

We accept all major types of insurance plans, including Medicare. We accept Providence at our McMinnville office location. Providence patients can all be seen by doctors Douglas Dawley and Bradley Evans at one of our Portland offices.  Currently we are unable to accept Kaiser health insurance. As a convenience to our patients, we will bill for both primary and secondary insurance coverage. Please be aware that patients ultimately are financially responsible for medical bills.

How do I reach my doctor there after hours?

If it’s an emergency, call 911. Otherwise, call the main office line for any of our four clinics, and you will be routed to our 24-hour answering service. One of our doctors is always on call.


What do I need to know about scheduling a test or procedure there?

Tests are scheduled by our clinic receptionists, either in person or by phone. If you’re being referred to us by an outside doctor, he or she will need to send the referral form to our office before the test can be scheduled. Further information will be given to you at the time of scheduling.

For procedures, the patients will be contacted by one of our procedure schedulers, who will set the time and location and provide further information.


How do I get test results from your office?

You can call the office where you received your test to get the results.


How do I get a prescription filled or refilled?

Contact your pharmacy and someone there will contact our office if needed.


How do I get my existing medical records transferred to your office?

You can either download the “NWCI Records Release” form and send that form to your other doctor. Or you can call your other doctor’s office and request that your medical records be sent to us.


Are your offices wheelchair accessible?

Yes, all four locations are wheelchair accessible.
